PORTRAIT AGREEMENT & MODEL RELEASE:
*I irrevocably and absolutely consent to the unrestricted use by Dana Ellis Photography and its successors, assigns and designees (“photographer”) and to those acting with its permission authority to use any and all photographic or other images of me that the Photographer creates/created or makes/made on date above for all purposes, in any form, and in any and all media, including, without limitation, advertising, solicitation, stock photography or trade, to copyright same in photographers own name or any other name that she may choose and the right to use my name in connection therewith if photographer so chooses.
*I waive any right to inspect or approve the finished Images, advertising copy, text, or other printed matter that may be used in conjunction therewith, or to inspect or approve the eventual uses to which the Images may be applied.
*I relase and discharge Photographer and those acting under its authority from any and all liabilities, claims and demands arising out of relating to any blurring, distorition or alteration whether intentional or otherwise, that may occur or be produced in connection with the images, or in connection with any processingm alteration, transmission, display or publication of the Images.
*This agreement constitues the sole, complete and exclusive agreement between Photographer and Model regarding the Images and Model is not relying on any other compensation whether oral or written. Model acknowledges that he/she has read this release and has received a copy of this release.
*By signing below you certify that You are at least 18 years of age. You have read this portrait agreement/contract and fully understand the contents thereof.
